Family Nursing and Home Care has officially launched a remote monitoring technology platform,' Luscii’.
It will run as a two-year pilot aimed at supporting patients to live safely and independently at home.
By enabling clinicians to monitor patients remotely, the programme is designed to identify early signs of deterioration and reduce the need for hospital visits.
It received a grant from Digital Jersey's CareTech program, which funds innovation in health and care.

Since its launch, over 30 patients have been actively using the service, receiving care from home rather than in hospital settings.
The pilot will initially focus on frailty and in the second year, the programme will expand to include Diabetes and Wound Care, with a further 100 patients expected to join.
